The first 10 bronze medallions were released in 1997 and sold individually in poly bag packaging.  In 1998, the Velociraptor medallion was released and the medallions were then sold individually in blister packs.  
 
NOTE: The dates on these medallions are confusing.  The first 10 medallions are essentially a reissue of the 1994 'Cool Mike's Mint Caps Bronze POGS' issue except descriptive information replaced the 'Cool Mike's Mint Slammer' logos.  For this reason, the fronts of the first 10 medallions have a 1994 date with a 'Hoffman & Hoffman" copywrite.  However, the reverse of most of the coins have a 1996 date which I believe was when the descriptive information was added.  But, there may be variations or even other Hoffman Mint sets.  For example, the T-rex medallion shown below has a 1994 on the reverse. Is this from a different set or was it perhaps the first of the medallions produced as a 'proof of concept' and the date was never changed when the medallion went into production?
 
Finally, the aforementioned Velociraptor medallion has a 1998 date and a 'Hoffman Mint' copywrite on the front.  I have not seen the reverse of this medallion, so I do not know what date, if any, is stamped there.
